Complete this form at the beginning of the year for any and all students who will be traveling to one of the listed events. Bring a copy with you to every event.
Complete and send to State Advisor prior to the first DECA trip to ensure the advisor has the above Consolidate Comprehensive Consent Form on file for each student attending the Missouri DECA activities.
This form must accompany all written event entries, be signed by student and teacher, to ensure written materials are the original work of the competing student(s).
