Complete this form at the beginning of the year for any and all students who will be traveling to one of the listed events.

Bring a copy with you to every event.

Complete and send to State Advisor prior to the first DECA trip to ensure the advisor has the above Consolidate Comprehensive Consent Form on file for each student attending the Missouri DECA activities.

This is an agreement by one advisor to help supervise students from another chapter to help meet student to teacher ratios at events/conferences.

Complete or update this form at the beginning of the school year and send to State Advisor.
